National Audit Office reports on quality - primarily ECO & GBIZ - but also LG SHF type schemes. We would recommend starting with the much shorter "Summary" doc (c.13 pages). BUT the Figure 9 chart of failure %'s (page 37 or 38 - depends how you count) is probably the key thing to look at in the whole report.
